This might be a timing issue triggered by the difference between an Alt-F1
boot and a normal boot.
I recommend you run the lastest testlog and see if it reports any
config.sys ordering issues.
Is OS2LVM.DMD the last DMD loaded in your config.sys? It often is and, if
so, this would also makes it the last BASEDEV loaded.
When you get a hang after loading the last BASEDEV, it usually indicates a
problem fully enabling the protect mode drivers that were loaded as
BASEDEVs. Unfortunately there are lot of seemingly minor configuration
issues that can cause this kind of hang.
David's testlog and Doug Bissett's lcss are good tools for verifying and
optimizing your config.sys.
