Here's the setup: You're gong to configure Port Address Translation (PAT), or what is commonly just called "port forwarding." You need to know the port(s) which will be required for the VoIP hardware. In your router, you simply configure one or more port forwards from, say, port 7000 to port 7000 at the IP address of your gateway device (I'm using 7000 as an example, obviously). You configure for DDNS (Dynamic DNS) with a regular service, such as DynDNS.org or no-ip.com (I know we've had this discussion before on-list). This will keep your hostname updated in DNS when the IP address shifts. On the outside, the client will have to be able to resolve DNS names (otherwise, you will require a static IP on your broadband connection in front of the gateway). Simply configure it to look for the gateway at mygatewayname.yourdomain.com (your dynamic DNS address). That's it. When the traffic hits your router over one of the forwarded ports, it is passed off to the gateway device behind it.
