I assume that "to" meant "two", but I see only provision for one --- doesn't "Name of TCPIP Domain" refer to to a host's domain name such as @home.com, attcable.com or something  similar that ISPs sometimes require (seems less used this day --- I know I never used it).

While you're correct that I've never seen more than three DNS addresses being given by an ISP to any one user, its still a useful fallback.  Back when Verizon.Net was still BellAtlantic.Net and I had a static IP address with them, their DNS servers were erratic (to say the least); the BA newsgroups consistently had messages about which servers were working best at the moment and everyone would change their settings according.  While each had been given 3 DNS addresses, I gather there were actually about 8-10 servers used by BA and the addresses you initially were given was determined by average load levels or whatever that day; I avoided the entire problem by using 2 addresses from BA and 1 from the university at which I was then assigned by the Army (still, would have preferred 3 BA and 2 University DNS addresses as I set up in Windows) which was also convenient in that my laptop did not have to be rebooted when moving between the two locations where I had different fixed I!
