Perhaps this note in the documentation explains why my card is not seen:
"PCI does NOT implement PCMCIA device detection!! PCI can only detect
CardBus cards that are inserted into CardBus controllers. PCI can NOT
detect PCMCIA cards, legacy-type PCMCIA controllers, or CardBus cards that
are plugged into PCMCIA controllers."
