Sorry to hear of your troubles, Mark. There are two (known) PCI ID's for the 2915: 4223 and 4224. In your GenMAC driver directory,  make sure you have the subdirectory:

11-15-06 10:27p         <DIR>      0 ----  WRAPPER_8086_4223

And in there, something similar to the following:

I believe it is identical to the file for the
4224 as is possibly the driver file.  It looks
like the developer was prototyping and left some
files incompletely configured.

Which build of GenMAC are you using?

STACK32 I've found to be important. In particular, when I first started experimenting, I could not load UNIAUD and GenMAC (for this card, at least) without it (I got the beep symptom Ed described and a hang  at boot).

I'm not familiar with that parameter.

Also for troubleshooting, try REMMING your USB statements (ISTR some issues tweaking those up at first).

I'll give it a try next time I have an
opportunity to install.
